RGSA Waiver and Release Agreement

Adult Waiver

(Waiver and Release of Liability, Assumption of Risk & Indemnity Agreement – Adult)

I, the undersigned participant, on behalf of myself, my personal representatives, heirs, and next of kin, do hereby agree to the following:

1. Acknowledgment of Risks

I acknowledge that participation in basketball, volleyball, pickleball, and other sports training activities involves inherent risks, including but not limited to physical injury, illness, property damage, and unforeseen hazards. I understand that participation may expose me to communicable diseases, including but not limited to COVID-19, and assume such risks.

2. Release of Liability

I hereby release, waive, and discharge Ronnie Grandison, Ronnie Grandison Sports Academy, LLC, and all related entities, officers, employees, agents, and affiliates (the “Releasees”) from any and all liability for injury, illness, death, or property damage arising from my participation in any training, instruction, or related activities, regardless of cause, including negligence of the Releasees.

3. Assumption of Responsibility

I assume full responsibility for any risks associated with my participation, whether foreseen or unforeseen, and whether caused by the negligence of the Releasees or otherwise. I agree to indemnify and hold harmless the Releasees from any claims, damages, or costs, including attorney fees, arising out of my participation.

4. Medical Authorization

In the event of an emergency, I authorize the Releasees to secure medical treatment for me. I understand that I am responsible for any medical costs incurred as a result of such treatment.

5. Severability & Governing Law

This Agreement is intended to be as broad and inclusive as permitted by the laws of the State of Ohio. If any portion is found invalid, the remaining provisions shall continue in full force and effect.

6. Media Release

I grant permission for Releasees to use my name, image, and likeness in promotional materials, including but not limited to television, radio, online media, social media, and print advertisements, without compensation or further consent.

7. Acknowledgment & Understanding

I have carefully read and fully understand this Waiver and Release of Liability. I acknowledge that by signing below, I am voluntarily giving up substantial legal rights on my own behalf.

Minor Waiver

I, for myself and on behalf of my minor child, my personal representatives, heirs, and next of kin, do hereby agree to the following:

1. Acknowledgment of Risks

I acknowledge that participation in basketball, volleyball, pickleball, and other sports training activities involves inherent risks, including but not limited to physical injury, illness, property damage, and unforeseen hazards. I understand that participation may expose my child to communicable diseases, including but not limited to COVID-19, and assume such risks.
